The role emphasizes strong communication with referral sources, patients, families, and internal teams, while ensuring compliance with regulations and timely data entry into the EHR system.

Qualifications

• Strong communication and interpersonal skills for interacting with referral sources, patients and families.
• Excellent organizational abilities to manage multiple referrals and deadlines.
• Attention to detail in documenting admissions and authorizations.
• Ability to coordinate with clinical, billing and coordination teams.
• Proficiency with Microsoft Office and Excel for data entry and reporting.

Responsibilities

• Review and evaluate referrals for accuracy and appropriateness for home health services.
• Verify eligibility based on payer sources and clinical needs.
• Coordinate with clinical staff to ensure appropriate services are available.
• Ensure all documentation and authorizations are in place before case acceptance.
• Serve as primary contact for referral sources and patients during onboarding.
