The Wonderful Company is seeking a Behavioral Health Therapist based in Lost Hills, CA, with travel to other Central Valley wellness centers. You will be part of an onsite primary care team, enhancing patient physical and mental health through integrated care and collaboration with a psychiatric consultant and other providers.

Responsibilities include coordinating mental health care, screening for disorders, and facilitating referrals.

Qualificações

• Licensed in California as an LCSW or LMFT.
• Fluent in Spanish, both spoken and written.
• Experience collaborating in a primary care/mental health team is preferred.

Responsabilidades

• Support and coordinate mental health care with the primary care team and other providers.
• Screen and assess patients for mental health, substance use, and social needs.
• Educate patients about mental health conditions and treatment options.
• Monitor symptoms and adverse effects; adjust care as needed.
• Facilitate referrals to community services and psychosocial treatments.
• Support psychotropic medication management and adherence.
• Provide brief evidence-based interventions (e.g., ACT, CBT concepts) in primary care.
• Document progress and outcomes in the medical registry and share with PCPs.
• Participate in weekly caseload consultations with psychiatric consultants.
