Job summary
The Wonderful Company is hiring a Behavioral Health Therapist in Lost Hills, CA, with travel to other Central Valley wellness centers. The role integrates into an enhanced Primary Care team with a psychiatric consultant and social service providers to improve patients' physical and mental health.
Licensed in CA (LCSW/LMFT) and Spanish-speaking are required. Travel to Lost Hills, Delano, and mobile clinics is expected.
Qualifications
- Licensed LCSW or LMFT in California.
- Spanish fluency required.
- Experience with adults, teens, and pediatric patients preferred.
- Knowledge of social service agencies, state regulations, and APA standards.
- Demonstrated ability to collaborate effectively in a team setting.
Responsibilities
- Coordinate mental health care with primary care team and other mental health providers.
- Screen and assess for mental health and social needs.
- Provide education on mental health and treatment options.
- Monitor symptoms and treatment side effects.
- Facilitate referrals to community services and specialty care.
- Support psychotropic medication management with adherence and safety.
- Conduct brief evidence-based interventions in primary care settings.
- Participate in weekly caseload consultations with psychiatric consultants.
