Job summary

The Wonderful Company in Lost Hills, CA is seeking a Behavioral Health Therapist to join our onsite primary care team. This role travels to other wellness centers in the Central Valley and collaborates with a PCP, a psychiatric consultant, and other providers to improve patients' physical and mental health.

Key duties include coordinating mental health care, screening for disorders, educating patients, monitoring symptoms, and facilitating referrals and treatment plans.

Qualifications

• Must hold a current/valid LCSW or LMFT license issued by the California Board of Behavioral Sciences.
• Spanish fluency is mandatory for this role.

Responsibilities

• Coordinate mental health care with patients' primary care providers and other treating mental health providers.
• Screen and assess patients for common mental health and substance use disorders.
• Provide patient education on mental health and treatment options.
• Monitor patients for changes in symptoms and treatment side effects.
• Facilitate referrals to psychosocial treatments and community resources.
• Support psychotropic medication management with adherence and side effect monitoring.
• Lead brief interventions using evidence-based techniques in primary care settings.
• Document progress and treatment recommendations in the registry.
