Job summary

The Wonderful Company is seeking a Behavioral Health Therapist based in Lost Hills, CA, with travel to Central Valley wellness centers. You will join an onsite primary care team to improve patients’ physical and mental health, coordinating care with PCPs, psychiatric consultants, and other providers.

Applicants must hold a CA LCSW or LMFT, speak Spanish fluently, and bring collaborative, patient-centered care.

Qualifications

• Current/valid LCSW or LMFT license issued by the State of California.
• Must speak Spanish fluently.
• Preferred experience with adults, teens, and pediatric patients.
• Knowledge of social service agencies, state regulations, and APA standards.
• Demonstrated ability to collaborate effectively in a team setting.
• Ability to maintain professional relationships with patients and team members.

Responsibilities

• Support and coordinate mental health care with PCP and other providers.
• Screen and assess for mental health and social needs.
• Provide patient education on disorders and treatments.
• Monitor symptoms and treatment side effects.
• Facilitate referrals to services outside the clinic.
• Support psychotropic medication management with PCPs.
• Provide brief evidence-based interventions (e.g., CBT, IPT).
• Engage in weekly caseload consultations with psychiatric staff.
• Track outcomes in the registry and document progress.
• Adjust treatment plans in consultation with PCP and psychiatrist.
• Travel to Lost Hills, Delano, Student Wellness Centers, and Mobile clinic.
