Job summary

The Wonderful Company seeks a Behavioral Health Therapist based in Lost Hills, CA, traveling to Central Valley wellness centers as part of the on-site primary care team.

You will screen for mental health needs, coordinate care with PCPs, provide brief interventions, manage referrals, and document progress in the clinic registry; travel to Lost Hills, Delano, Student Wellness, and Mobile clinics is required.

Qualifications

• Current/valid LCSW or LMFT license issued by the State of California Board of Behavioral Sciences.
• Must speak Spanish fluently.
• Preferred experience with adults, teens, and pediatric patients.
• Knowledge of social service agencies, state regulations, and APA standards.
• Demonstrated ability to collaborate effectively in a team setting.
• Ability to maintain effective and professional relationships with patients and other team members.

Responsibilities

• Coordinate mental health care with the patient’s PCP and other providers.
• Screen and assess patients for mental health, substance use, and unmet social needs.
• Educate patients about mental health and treatment options.
• Monitor patients for changes in symptoms and medication effects.
• Develop relationships with community providers for referrals outside the clinic.
• Support psychotropic medication management with PCPs.
• Provide brief, evidence-based interventions in primary care settings.
• Facilitate in-clinic or outside referrals for psychosocial treatments.
• Participate in regular caseload consultations with the psychiatric consultant.
• Track patient follow-up and outcomes in the registry and document progress.
• Travel to Lost Hills, Delano, Student Wellness Centers, and Mobile clinic.
