Job summary

Elm & Oak Health seeks a Team Lead for Safe Options Support (SOS) teams serving individuals experiencing homelessness across Onondaga and Oswego Counties in New York. The role requires guiding a multidisciplinary staff of care managers, peer specialists, and clinicians, with travel across the counties and flexible hours.

You will coach staff, develop partnerships with hospitals, law enforcement, and housing providers, and ensure 24/7 crisis response while maintaining regulatory and contractual

Qualifications

• Bachelor’s Degree or AAS with related experience.
• Minimum 5 years in case management or clinically related area.
• Minimum 2 years working with homeless or precariously housed populations.
• Knowledge of counseling principles for mental illness and substance use disorders.
• Knowledge of crisis management and de-escalation techniques.

Responsibilities

• Direct supervision, coaching and education of staff (50%).
• Department operational oversight (20%).
• Quality and compliance oversight (20%).
• Training and development (10%).
• Coordinate with SOS, Health Home Care Management and OMH and community providers.
