Elm & Oak Health seeks a Team Lead for Safe Options Support (SOS) teams serving individuals experiencing homelessness across Onondaga and Oswego Counties in New York. The role requires guiding a multidisciplinary staff of care managers, peer specialists, and clinicians, with travel across the counties and flexible hours.

You will coach staff, develop partnerships with hospitals, law enforcement, and housing providers, and ensure 24/7 crisis response while maintaining regulatory and contractual

Qualifications

  • Bachelor’s Degree or AAS with related experience.
  • Minimum 5 years in case management or clinically related area.
  • Minimum 2 years working with homeless or precariously housed populations.
  • Knowledge of counseling principles for mental illness and substance use disorders.
  • Knowledge of crisis management and de-escalation techniques.

Elm & Oak Health is on a mission to bring dignity and stability to the most vulnerable members of our communities. We're looking for a Team Lead to guide our Safe Options Support (SOS) teams serving individuals experiencing homelessness across the Onondaga/Oswego County areas.

You will lead and mentor a multidisciplinary team of care managers, peer specialists, and clinicians, coaching your people, forging strong partnerships with hospitals, law enforcement, and housing providers. The role requires significant travel throughout Onondaga and Oswego Counties and a willingness to work flexible hours.

ESSENTIAL JOB DUTIES
Direct Supervision, Coaching/Education Training Staff (50%)
  • Provide mentoring, guidance and coaching to staff regarding policies and procedures.
  • Provide mentoring, guidance and coaching regarding service delivery and interventions.
  • Assist staff with Care Management referrals and network development.
  • Promote a positive working environment through constructive reinforcement and recognition.
  • Conduct interim and yearly staff evaluations with Manager.
  • Manage recruitment and address staff performance issues per policies.
  • Coordinate weekly staff schedules.
Department Operational Oversight (20%)
  • Manage day‑to‑day operations in compliance with internal and external standards.
  • Coordinate program outreach at community hotspots and during hospital admissions.
  • Develop partnerships with local hospitals, law enforcement, shelters, and housing providers.
  • Facilitate case reviews for members at risk and document recommendations.
  • Ensure 24/7 crisis response through on‑call requirements.
  • Work with the Single Point of Access department on enrollment and reporting.
  • Engage all stakeholders with respect and cultural sensitivity.
Quality and Compliance Oversight (20%)
  • Oversee creation of a multi‑disciplinary SOS community support team with OMH and regional partners.
  • Attend OMH and regional collaboration meetings.
  • Conduct internal audits and case record reviews, report findings to leadership.
  • Assist in quality improvement projects addressing barriers to care.
  • Ensure regulatory and contract standards are met.
  • Monitor compliance monthly, including HIPAA.
  • Develop policies and procedures for the SOS CTI program.
  • Support audit processes and follow up on deficiencies.
Training & Development (10%)
  • Develop, implement, and monitor training programs.
  • Conduct orientation and continuing education sessions.
  • Create training materials and evaluation processes.
  • Plan education activities and manage budget for resources.
  • Act as liaison between SOS, Health Home Care Management, and other departments.
  • Act as liaison with OMH and community providers.
Other Functions and Responsibilities
  • Demonstrate cultural awareness, knowledge, and skills.
  • Attend team meetings and organizational/departmental trainings.
  • Perform other duties as assigned.
Position Limitations
  • Cannot perform licensed or registered tasks such as diagnosis or medication recommendations.
  • Cannot transport active Elm & Oak Health members.
  • Cannot perform hands‑on care.
Minimum Requirements / Licenses / Certifications
  • Bachelor’s Degree or AAS with 2 years equivalent experience.
  • Minimum 5 years in case management or a clinically related area.
  • Minimum 2 years working with homeless or precariously housed populations.
  • Knowledge of counseling principles for mental illness and substance use disorders.
  • Knowledge of treatment, rehabilitation and community support programs.
  • Knowledge of crisis management and de‑escalation techniques.
  • Ability to develop, evaluate, and modify treatment interventions.
  • Strong leadership and organizational skills.
  • Excellent planning, organization, and implementation skills.
  • Excellent oral and written communication skills.
  • Excellent computer skills and mastery of SOS program software.
  • Commitment to our member population.
  • Valid NYS driver’s license and reliable transportation for travel.
Preferred Qualifications
  • Previous leadership or supervisory experience.
  • Collaborative style with strong team building and mentoring skills.
  • Experience working with individuals with mental health issues.
