Job summary

St. Croix Hospice in Oshkosh, WI seeks a compassionate Social Worker to join our patient-care team. You will meet with patients and families, perform psychosocial assessments, coordinate resources, and support care planning in line with the Plan of Care.

The role includes bereavement support and collaboration with an interdisciplinary team. A valid social work license and driving eligibility are required, with standard full-time hours.

Qualifications

• Bachelor’s degree in social work from a CSWE-accredited program.
• Current Social Work license in practicing state(s).
• Five licensure levels: CSW, CAPSW, CISW, LCSW, SWTC.
• Ability to pass background, consumer report, and drug testing.
• Conduct psychosocial assessments and support planning for patients and families.
• Provide counseling, crisis intervention, and bereavement support.
• Collaborate with the interdisciplinary team and maintain HIPAA-compliant documentation.
• Require reliable transportation, valid driver's license, and auto insurance.

Responsibilities

• Coordinate with clinical team and community resources for services.
