Job summary

St. Croix Hospice seeks a compassionate Social Worker to join the patient care team, providing emotional support and practical resources to patients and families according to each Plan of Care.

You will meet with families, collaborate with clinicians, and help navigate community services while maintaining HIPAA compliance. Requirements include a 4-year degree in social work, an active state license, and the ability to coordinate care plans and bereavement activities.

Qualifications

• Must have a BSW and state social work licensure.
• Current Social Work license in state(s) of practice.
• HIPAA compliance and accurate documentation are required.
