Job summary

St. Croix Hospice is seeking a Social Worker to support patients and families with emotional and practical care aligned with each Plan of Care. The role collaborates closely with the clinical team and connects families to community resources.

The position involves psychosocial assessments, counseling, crisis intervention, and bereavement support, with requirements including a CSWE-accredited BSW and state licensure. A valid driver’s license and reliable transportation are required.

Qualifications

• Bachelor’s degree in social work from CSWE-accredited institution.
• Current Social Work license in state(s) of practice.
• Three licensure levels: LBSW, LSW, LCSW.
• Ability to pass background, consumer report, and drug test.
• Conduct psychosocial assessments and care planning support.
• Provide counseling, crisis intervention, and bereavement support.
• Collaborate with interdisciplinary team and maintain HIPAA-compliant documentation.
• Reliable transportation, valid driver’s license, and proof of auto insurance.

At St. Croix Hospice, the Social Worker is an essential member of the patient care team, providing compassionate emotional and practical support to patients and their families in alignment with each individual’s Plan of Care. In this role, the Social Worker meets with patients and their loved ones, collaborates closely with the clinical team, and connects families with appropriate community resources and services.

Social Worker Requirements & Core Responsibilities:

• Bachelor’s degree in social work from an institution accredited by the Council on Social Work Education.
• Current Social Work licensed in the state(s) of practice
• Three (3) licensure levels:
• Licensed Bachelor Social Work (LBSW)
• Licensed Social Worker (LSW)
• Licensed Clinical Social Worker (LCSW)
• Ability to pass background, consumer report, and drug test.
• Conduct psychosocial assessments, identify safety or support needs, and provide resources, referrals, and care planning support for patients and families.
• Provide counseling, crisis intervention, and bereavement support, including follow up for at risk individuals and coordination of bereavement activities.
• Collaborate with the interdisciplinary team, maintain accurate documentation, support cultural and end of life needs, liaise with community agencies, and ensure HIPAA compliance.
• Must have reliable transportation, valid driver’s license, and proof of auto insurance
