Job summary

St. Croix Hospice seeks a compassionate Social Worker to support patients and families, perform psychosocial assessments, and coordinate care with the clinical team. You will help access community resources and plan care in alignment with each patient’s goals.

Qualifications

  • Bachelor’s degree in social work from a CSWE-accredited program.
  • Current social work license in practice state(s).
  • Ability to pass background, consumer report, and drug tests.

This position involves meeting with patients and their loved ones, working collaboratively with the clinical team, and helping families access the appropriate community resources and services they need.If you're a compassionate Social Worker who values meaningful patient connections and autonomy in your practice, we’d love to meet you.Social Worker Requirements & Core Responsibilities:Bachelor’s degree in social work from an institution accredited by the Council on Social Work Education.Current Social Work licensed in the state(s) of practiceFive (5) licensure levels:Certified Social Worker (CSW)Certified Advanced Practice Social Worker (CAPSW)Certified Independent Social Worker (CISW)Licensed Clinical Social Worker (LCSW)Social Work Training Certificate (SWTC)Ability to pass background, consumer report, and drug test.Conduct psychosocial assessments, identify safety or support needs, and provide resources, referrals, and care planning support for patients and families.Provide counseling, crisis intervention, and bereavement support, including follow up for at risk individuals and coordination of bereavement activities.Collaborate with the interdisciplinary team, maintain accurate documentation, support cultural and end of life needs, liaise with community agencies, and ensure HIPAA compliance.Must have reliable transportation, valid driver’s license, and proof of auto insuranceSocial Worker Benefits & Schedule:Schedule: M-F 8:00am to 5:00pm, Full-time position.Health, dental, vision, and life insurance.Company-paid short/long-term disability and basic life insurance.401(k) with up to 4% company match.Tuition reimbursement and professional development opportunities.Paid time off and paid holidays.
