Caring Heart Rehabilitation & Nursing Center in Philadelphia, PA is currently hiring for a full time Social Worker.

Social Worker experience in long term care is highly preferred. This role involves planning social services for adult and geriatric residents, evaluating admission inquiries, and ensuring appropriate facility placement while addressing therapeutic, neurological and dementia needs as applicable.

Qualifications

• Experience in long term care preferred.
• Knowledge of age-specific developmental factors for adult and geriatric residents in planning social services.
• Ability to evaluate admission inquiries and determine appropriateness of facility placement.

Responsabilités

• Demonstrates knowledge of age-specific developmental factors to plan social services for adult and geriatric residents.
• Plans social services addressing needs such as therapeutic, neurological and dementia care, dialysis, IV therapy, and infectious diseases where applicable.
• Follows up on admission inquiries and assesses prospective residents for facility placement.

Job Duties and Responsibilities of the Social Worker:

• Demonstrates knowledge of age specific developmental factors specific to adult and geriatric residents (i.e. physical, cognitive, and socialization factors) in planning social services programs.

• Demonstrates knowledge, skills, and techniques necessary to plan social services programs for residents with the following needs: therapeutic, neurological and dementia, dialysis, IV therapy, and infectious diseases if applicable.

• Promptly follows-up on admission inquiries, evaluates and assesses the needs of prospective residents to determine appropriateness of facility placement
