Job summary

WeCare Centers is seeking a compassionate Social Worker to join our skilled nursing team in Kansas. This role supports residents and families by coordinating psychosocial assessments, care planning, discharge planning and community resources to promote resident-centered care.

Qualifications

• Degree in Social Work or related field as required by applicable regulations.
• Minimum 1 year of Skilled Nursing/Long-Term Care experience preferred.
• Strong communication and organizational skills.
