Job summary

WeCare Centers is seeking a compassionate Social Worker to join our skilled nursing team in Kansas. This role supports residents and families by coordinating psychosocial assessments, care planning, discharge planning and community resources to promote resident-centered care.

Applicants should have a degree in Social Work and at least 1 year experience in skilled nursing or long-term care; PCC experience is a plus. We offer competitive compensation and an inclusive, supportive workplace.
