Job summary

Overland Park SNF Healthcare, LLC is seeking a Social Worker to support residents and their families, coordinating social services to improve wellbeing and independence. The role requires a Bachelor’s in Social Work or related field with 2 years of supervised healthcare experience.

Qualifications

• Bachelor’s Degree in Social Work or in Human Services with 2 years of supervised social work in a health care setting.
• MSW preferred.
• Ability to read and comprehend policy and procedure manuals.
• Ability to present information and respond to questions from managers and employees.

Responsibilities

• Assist residents in achieving the highest practicable level of self-care, independence and well-being.
• Provide emotionally supportive social services to promote physical, mental, and psychosocial wellbeing.
• Develop care plans for social services issues and assess residents’ needs.
• Coordinate with interdisciplinary team to deliver social services and discharge planning.
