Trinity Health’s Nazareth Hospital in Philadelphia is seeking a Social Worker (PRN, part-time) for day shifts. The role involves collaboration with Care Coordination, discharge planning, and patient advocacy to meet health care needs. Requires a Bachelor’s in Social Work; Master’s preferred, with hospital experience.

Responsibilities

• Provides support and counseling to patients and families adjusting to illness, diagnoses, or changes in living situations.
• Documents interventions and outcomes in the electronic record.
• Participates in multidisciplinary rounds with the care team.
• Performs assessments to identify gaps in care, develop goals, and facilitate interventions.

Description

Job Title: Social Worker

Employment Type: PRN (Weekday and Weekend availability 8am to 4:30pm, required to work 2shifts/month, 2 holiday/yr.)

Location: St. Mary’s Medical Center and Nazareth Hospital (Required)

Position Purpose

In collaboration with Care Coordination staff, this position is directly involved in the process of assessment, planning, facilitation, and advocacy for options and services to meet individual patient’s health care needs. Through communication and identification of available resources, participates in the promotion of quality outcomes, patient satisfaction, and cost-effective care delivery. This position reports to the Manager of Care Coordination.

As a “Social Worker” You Will

• Recognizes special needs/concerns of populations served and adjusts communication accordingly, i.e., language barrier, diminished hearing, concrete terms, etc.
• Collaborates with the Care Coordination staff and multidisciplinary team members regarding discharge planning and arrange for needed care in the home or an alternate setting.
• Provides support and counseling to patients/families experiencing and/or anticipating issues to adjusting to illness, catastrophic diagnoses, changes in living situations and bereavement. Establish a plan. Documents interventions and outcomes as required in the electronic record.
• Actively participates and provides pertinent information in multidisciplinary rounds on assigned patients.
• Completes appropriate assessments for assigned patients to identify gaps in care, develop goals, and facilitate interventions and resources.
• Ability to perform diversified duties with time limitations with a high degree of accuracy
• Must be able to work independently and on a team.
• Demonstrates ability to use problem solving, critical thinking and priority setting skills

Minimum Qualifications

• Bachelor's Degree in Social Work/Services
• Master's Degree (preferred)
• Minimum of one (1) year experience in a hospital or health care facility

Non-benefit eligible Roles

• Under 0.3 FTE (16 hours weekly) or per diem roles**
