Trinity Health’s Nazareth Hospital and St. Mary’s Medical Center in Philadelphia are seeking a Part-time Social Worker. The role supports Care Coordination with assessment, planning, advocacy, and resource identification to meet patients’ health care needs.

The position requires collaboration with a multidisciplinary team, discharge planning, and counseling for patients and families. Minimum Bachelor’s in Social Work; Master’s preferred; hospital experience preferred.

Qualifications

• Bachelor's Degree in Social Work/Services.
• Master's Degree preferred.
• Minimum of one year experience in a hospital or health care facility.

Responsibilities

• Collaborates with Care Coordination staff and multidisciplinary team on discharge planning and arranging home or alternate care.
• Provides support and counseling to patients and families adjusting to illness, bereavement or changes in living situation.
• Documents interventions and outcomes in the electronic record.
• Participates in multidisciplinary rounds and contributes relevant information for assigned patients.
• Performs assessments to identify gaps in care, develop goals, and facilitate interventions.

Description:

Job Title: Social Worker

Employment Type: PRN (Weekday and Weekend availability 8am to 4:30pm, required to work 2shifts/month, 2 holiday/yr.)

Location: St. Mary’s Medical Center and Nazareth Hospital (Required)

Position Purpose

In collaboration with Care Coordination staff, this position is directly involved in the process of assessment, planning, facilitation, and advocacy for options and services to meet individual patient’s health care needs. Through communication and identification of available resources, participates in the promotion of quality outcomes, patient satisfaction, and cost-effective care delivery. This position reports to the Manager of Care Coordination.

As a “Social Worker” you will:

• Recognizes special needs/concerns of populations served and adjusts communication accordingly, i.e., language barrier, diminished hearing, concrete terms, etc.


• Collaborates with the Care Coordination staff and multidisciplinary team members regarding discharge planning and arrange for needed care in the home or an alternate setting.


• Provides support and counseling to patients/families experiencing and/or anticipating issues to adjusting to illness, catastrophic diagnoses, changes in living situations and bereavement. Establish a plan. Documents interventions and outcomes as required in the electronic record.


• Actively participates and provides pertinent information in multidisciplinary rounds on assigned patients.


• Completes appropriate assessments for assigned patients to identify gaps in care, develop goals, and facilitate interventions and resources.


• Ability to perform diversified duties with time limitations with a high degree of accuracy


• Must be able to work independently and on a team.


• Demonstrates ability to use problem solving, critical thinking and priority setting skills

Minimum Qualifications:

• Bachelor's Degree in Social Work/Services


• Master's Degree (preferred)


• Minimum of one (1) year experience in a hospital or health care facility
