Saint Francis Hospital in Wilmington, DE is seeking a Social Worker - Acute Care to monitor patient progress, coordinate discharge planning, and document findings for quality care and efficient transitions.

The role requires an MSW and at least 1 year of acute care experience, with strong communication and leadership skills. This is a full-time day shift position on-site, with on-call responsibilities one week per month.

Qualifications

  • Masters in Social Work (MSW) is required.
  • At least 1 year of acute care social work experience.
  • Strong leadership and excellent interpersonal and communication abilities.

Responsibilities

  • Prioritizes and facilitates timely discharge planning for post-acute care needs.
  • Maintains documentation in medical records and Midas.
  • Interviews patients and families to identify social needs for service planning.
  • Collaborates with the multidisciplinary team on discharge planning and care transitions.
  • Assesses for child abuse, domestic violence, or related concerns and coordinates referrals.
  • Completes assessments to identify gaps in care and set goals.
  • Assists with Medicaid applications and placement planning in facilities or home care.
  • Facilitates meetings with patients, families, and providers to plan care transitions.
  • Identifies risk factors for rehospitalization and helps mitigate barriers to discharge.
  • Maintains records according to regulations and organizational policies.

Employment Type: Full-time

Shift: Day Shift, Monday - Friday (8:00 am - 4:30 pm, on call from 4:30pm to 8am one week out of the month)
Location: Saint Francis Hospital - Wilmington, DE

Position Purpose

The Social Worker monitors the patient’s progress throughout hospitalization and documents relevant findings with regard to Social Services and discharge planning in a timely manner. Responsibilities as follows: Establish accountability for data-driven quality care, service and financial outcomes related to patient transitions of care. Set priorities for improving processes and outcomes related to patient care transitions. Consider outcomes focusing on the appropriateness, efficacy, efficiency, timeliness, accessibility, continuity of care, the safety of the care environment and respect of the patient’s personal values, preferences and beliefs.

As a Social Worker, you will:
  • Prioritizes and Facilitates discharge planning in a timely manner for those with post-acute care needs in order to avoid unnecessary delays
  • Maintains documentation in medical record and Midas.
  • Interviews patients/significant others within one working day of referral to obtain data on personal, social, medical, emotional, cultural, and religious history in order to delineate problems requiring Social Work intervention and to plan appropriate services.
  • Collaborates with the multidisciplinary team members regarding discharge planning and arranges for needed care in the home or an alternate setting.
  • Consults, assesses, refers, coordinates interventions in cases of suspected or actual situations of child or abuse and neglect or other forms of domestic violence and/or sexual abuse.
  • Completes appropriate assessments for assigned patients to identify gaps in care, develop goals, and facilitate interventions and resources.
  • Assists patients/families to complete applications for Medicaid benefits.
  • Manages the placement process into inpatient facilities. Reviews financial resources for long term placement and provides education to the patient/family. Also develops plans for transfer to SNF, acute rehabilitation facilities, assisted living, inpatient substance abuse centers and inpatient psychiatric hospitals.
  • Facilitates patient/family and provider meetings as necessary to develop and plan strategies related to psychosocial and economic issues, particularly those related to care progression and transition. May act as the liaison between patients/families and all providers of care within and external to the organization on social issues.
  • Works collaboratively with the multidisciplinary team to identify patients at risk for rehospitalization, and mediates barriers to successful discharge plan.
  • Works collaboratively with the Multidisciplinary team in assisting individual patients and their families to maximize function and independence by researching, accessing, or referring appropriate services and community agencies.
  • Assists the multidisciplinary team to assess psychological and social aspects of patient care needs, and collaborates on plan of care interventions.
  • Prepares and maintains individual patient and departmental records according to department, federal and state regulations; completes statistical information and prepares correspondence as needed.
  • Adheres to Discharge Planning and Organizational policies and standards as well as standards from external regulatory agencies and accrediting bodies,( e.g., JCAHO, Department of Public Health)
Minimum Qualifications
  • Candidate must have a Masters in Social Work (MSW)
  • At least 1 year experience providing social work services in an acute care setting required.
  • Strong leadership ability and excellent interpersonal skills, as well as, solid oral and written communication skills.
