St. Mary Medical Center in Langhorne, PA, is seeking a Social Worker for a PRN, part-time role with 16 hours per week. The position supports Care Coordination, with a Day Shift at St.

Mary’s Medical Center and Nazareth Hospital. The Social Worker collaborates with the care team to assess needs, plan discharges, and connect patients with relevant resources. The role requires a Bachelor’s in Social Work (Master’s preferred) and at least one year in a hospital setting.

Qualifications

Responsibilities

• Collaborates with Care Coordination and multidisciplinary team on discharge planning.
• Provides support and counseling to patients/families adjusting to illness or bereavement.
• Completes assessments to identify gaps in care and facilitates interventions and resources.
• Participates in multidisciplinary rounds and documents interventions in the medical record.
• Works independently and as part of a team with accuracy and prioritization.

Job Title: Social Worker

Employment Type: PRN (Weekday and Weekend availability 8am to 4:30pm, required to work 2shifts/month, 2 holiday/yr.)

Position Purpose

In collaboration with Care Coordination staff, this position is directly involved in the process of assessment, planning, facilitation, and advocacy for options and services to meet individual patient’s health care needs. Through communication and identification of available resources, participates in the promotion of quality outcomes, patient satisfaction, and cost-effective care delivery. This position reports to the Manager of Care Coordination.

As a “Social Worker” you will:

• Recognizes special needs/concerns of populations served and adjusts communication accordingly, i.e., language barrier, diminished hearing, concrete terms, etc.


• Collaborates with the Care Coordination staff and multidisciplinary team members regarding discharge planning and arrange for needed care in the home or an alternate setting.


• Provides support and counseling to patients/families experiencing and/or anticipating issues to adjusting to illness, catastrophic diagnoses, changes in living situations and bereavement. Establish a plan. Documents interventions and outcomes as required in the electronic record.


• Actively participates and provides pertinent information in multidisciplinary rounds on assigned patients.


• Completes appropriate assessments for assigned patients to identify gaps in care, develop goals, and facilitate interventions and resources.


• Ability to perform diversified duties with time limitations with a high degree of accuracy


• Must be able to work independently and on a team.


• Demonstrates ability to use problem solving, critical thinking and priority setting skills

Minimum Qualifications:

• Bachelor's Degree in Social Work/Services


• Master's Degree (preferred)


• Minimum of one (1) year experience in a hospital or health care facility

Non-benefit eligible Roles
