Job summary

Glacier Hills, part of Trinity Health Senior Communities, is seeking a Part-time Day Shift Social Worker to support residents in our Skilled Nursing community in Ann Arbor, Michigan.

As an LSW, you will assess needs, plan care, coordinate resources, and provide emotional support to residents and families, working with an interdisciplinary team to advocate for resident well-being and smooth transitions.

Qualifications

• Licensed Social Worker (LSW) in Michigan.
• Bachelor's or Master's degree in Social Work or related human services field.
• Healthcare social work experience preferred; experience with older adults or long-term care preferred.
• Compassionate, resident-centered approach and strong communication skills.

In this role, you'll build trusting relationships with residents and families, helping them navigate important transitions and providing support when it matters most. You'll work alongside a caring interdisciplinary team to advocate for resident needs, coordinate care, and ensure each individual receives the resources and guidance needed to thrive.

What You'll Do:

• Assist with resident assessments, care planning, and discharge planning.
• Conduct cognitive and mood evaluations and coordinate follow-up services as needed.
• Provide emotional support, guidance, and education to residents and families.
• Connect residents with community resources, financial assistance programs, and social service agencies.
• Maintain timely documentation and collaborate with the care team to support resident well-being.

What You'll Bring:

• Licensed Social Worker (LSW) in the State of Michigan.
• Bachelor's or Master's degree in Social Work or a related human services field.
• Healthcare social work experience preferred.Experience working with older adults or in long-term care preferred.
