Job summary

Trinity Health Senior Communities in Michigan seeks a full-time Social Worker to support residents and families, coordinate discharge plans, and develop individualized social care plans. The role requires a bachelor’s or master’s degree in social work or related field, Michigan licensure, and at least one year of supervised social work experience in a healthcare setting, preferably in long-term care.

Responsibilities

• Plan with residents, families and community interests; assist in discharge planning and social care plans; complete forms and coordinate with departments.
• Promote safe and appropriate placement on discharge; maintain ongoing communication with residents and stakeholders.
• Provide Medicare/Medicaid information and assist with financial programs; advise staff and agencies to resolve resident concerns.

Job Responsibilities

• Participate in planning related to resident, family, and community interests. Participate in discharge planning, development and implementation of social care plans and resident assessments. Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, arranging transportation, etc., as necessary, coordinating activities with other departments as necessary.
• Promotes optimum transition of resident for appropriate and safe placement for discharge. Provides ongoing communication with resident and responsible persons of progress/status during stay.
• Provide information to the resident/family on Medicare/Medicaid as well as other financial assistance programs available. Provide consultation to staff, community agencies, etc., in efforts to solve resident concerns through the development of social service programs.

Minimum Qualifications

• Must possess a bachelor's or master's degree in social work or a bachelor's or master's degree in a human services field ( i.e. sociology, special education, rehabilitation counseling and psychology.)

• One year of supervised social work experience in a healthcare setting working directly with individuals.
• Experience in long-term care, working with the senior population and dementia counseling.
• Must be licensed as a social worker in Michigan.
• Must lead by example and possess superior customer service skills and professionalism.
• Flexibility to adapt to ongoing change and work in a fast-paced, customer driven environment.
