Job summary

Glacier Hills is seeking a compassionate Part-Time Day Shift Social Worker to support our Skilled Nursing residents. As part of an interdisciplinary team, you will assess needs, coordinate discharge planning, and connect residents with community resources while documenting care and advocating for resident well‑being.

Ideal candidates hold an LSW in Michigan with a relevant degree and prior healthcare social work or long-term care experience.

Qualifications

• Licensed Social Worker (LSW) in Michigan.
• Bachelor's or Master's in Social Work or related field.
• Healthcare social work experience preferred.
• Experience with older adults or long-term care preferred.

Responsibilities

• Assist with resident assessments, care planning, and discharge planning.
• Conduct cognitive and mood evaluations and coordinate follow-up services.
• Provide emotional support, guidance, and education to residents and families.
• Connect residents with community resources, financial assistance programs, and social service agencies.
• Maintain timely documentation and collaborate with the care team to support resident well-being.

What You'll Do:
