Glacier Hills invites applications for a Part-Time Day Shift Social Worker in our Skilled Nursing community. Join our compassionate interdisciplinary team to support older adults and their families as they navigate transitions and access essential resources.

You will conduct assessments, coordinate care, and provide emotional support and guidance to residents and families. LSW licensure in Michigan and a relevant degree are required; healthcare social work and long-term care experience are

Part-Time Day Shift Social Worker

Days (32 hours per pay period)

In this role, you'll build trusting relationships with residents and families, helping them navigate important transitions and providing support when it matters most. You'll work alongside a caring interdisciplinary team to advocate for resident needs, coordinate care, and ensure each individual receives the resources and guidance needed to thrive.

What You'll Do:
