Job summary

Glacier Hills is seeking a compassionate Social Worker to join our Skilled Nursing team. This part-time Day Shift (32 hours per pay period) role focuses on resident assessments, care planning, discharge planning, and supporting families.

You will conduct cognitive and mood evaluations, connect residents with community resources, and collaborate with an interdisciplinary team to advocate for resident needs. LSW in Michigan required; Bachelor's or Master's in Social Work; experience with older

Qualifications

Licensed Social Worker (LSW) in Michigan.
Bachelor's or Master's degree in Social Work or related human services field.
Healthcare social work experience preferred.
Experience with older adults or in long-term care preferred.
Strong communication skills with a resident-centered approach.

Responsibilities

Assist with resident assessments, care planning, and discharge planning.
Conduct cognitive and mood evaluations and coordinate follow-up services.
Provide emotional support, guidance, and education to residents and families.
Connect residents with community resources, financial assistance programs, and social service agencies.
Maintain timely documentation and collaborate with the care team to support resident well-being.
