Alliance Health & Human Services invites a Full-Time Licensed Social Worker to join Navigator Homes on Martha’s Vineyard. You will help meet residents’ psychosocial needs and collaborate with the interdisciplinary team to develop individualized care plans that maximize health, dignity, and quality of life.

The ideal candidate holds MA LCSW, 2–3 years in long-term care, and strong communication and computer skills, with knowledge of EMR, MDS, and OBRA regulations.

Qualifikationen

• MA LCSW required.
• A minimum of 2-3 years' experience in a long-term care setting.
• Solid working knowledge of discharge planning, Medicare/Medicaid, OBRA regulations, community resources, social work methods and techniques.
• Excellent communication, interpersonal, and computer skills required.
• Knowledge of electronic medical records, MDS, and Care Plans.

Looking for a Full Time Licensed Social Worker for Navigator Homes

Navigator Homes on Martha's Vinyard provides all the tools necessary for you to excel in your role, including supportive management, good staffing levels, professional development, and an inclusive work environment. This position reports to Alliance Health Executive Director.
