Cape Cod Post Acute and Healthcare Center in Brewster, MA is seeking a full-time Social Worker to assess residents' psychosocial needs, develop care plans, and coordinate services to promote wellbeing.
Bachelor’s degree is required; Master’s degree is preferred. Minimum 1+ year experience in healthcare social work with strong communication and teamwork skills is expected, in a nursing home or rehab setting.
