Alliance Health & Human Services invites a Full-Time Licensed Social Worker to join Navigator Homes on Martha’s Vineyard. You will help meet residents’ psychosocial needs and collaborate with the interdisciplinary team to develop individualized care plans that maximize health, dignity, and quality of life.

The ideal candidate holds MA LCSW, 2–3 years in long-term care, and strong communication and computer skills, with knowledge of EMR, MDS, and OBRA regulations.

Formación

• MA LCSW required.
• A minimum of 2-3 years' experience in a long-term care setting.
• Solid working knowledge of discharge planning, Medicare/Medicaid, OBRA regulations, community resources, social work methods and techniques.
• Excellent communication, interpersonal, and computer skills required.
• Knowledge of electronic medical records, MDS, and Care Plans.

Responsabilidades

• Assists in meeting the psychosocial needs of residents in a long-term care facility.
• Collaborates with the inter-disciplinary team to develop and carry out individualized care plans to enable each resident to achieve their optimal level of health and well-being and maximize their quality of life.
