Job summary

Andwell Health Partners seeks a Hospice Registered Nurse to provide skilled, compassionate care in patients' homes across Farmington, ME and surrounding towns. You will assess, plan, and implement care that preserves dignity and comfort during the hospice journey.

You will educate families, collaborate with an interdisciplinary team, and advocate for patient-centered goals of care while delivering empathetic support in meaningful moments.

Qualifications

• Maine RN license (active) required.
• Minimum 1 year of recent clinical nursing experience.
• Experience in pain management and symptom control preferred.

Responsibilities

• Deliver skilled nursing services in patients' homes following physician-directed plans of care.
• Assess physical, emotional, psychosocial, and spiritual needs of patients and families.
• Develop individualized care plans with measurable outcomes with patients and families.
• Manage pain and symptoms to maximize comfort and quality of life.
• Educate patients, caregivers, and family members to support informed decisions.
• Advocate for patient rights and goals of care; collaborate with hospice team.
