Community Healthcare of Texas in Fort Worth, TX is seeking a Social Worker to support patients and families facing serious illness within hospice and palliative care services. You will collaborate on plans of care, document visits, and coordinate referrals to community resources.

Bachelor of Social Work with 1 year of applicable experience required; hospice or healthcare experience preferred. Working hours Monday to Friday, 8:00am–5:00pm.

Qualifications

• Bachelor of Social Work with 1 year of applicable experience required.
• Hospice or healthcare experience preferred.
• Proficiency with computer documentation and record-keeping.
