Job summary

Community Healthcare of Texas is recruiting a Social Worker to provide therapeutic support, education, and resources to patients and families facing serious illness. You will advocate for patient wishes within the care team and contribute to the Plan of Care while documenting visits and coordinating with community resources.

Qualifications include a Master’s degree in Social Work; reliable transportation and a driver’s license; hospice or healthcare experience preferred; and proficient computer

POSITION SUMMARY

To provide therapeutic/emotional support, education, and resource information to patients and their families; Advocate for patients’ needs/wishes with team members, family members, caregivers, and other providers.
