Job summary

Community Healthcare of Texas is currently seeking a Social Worker to provide therapeutic support and resource information to patients and families facing end-of-life issues. The position involves advocating for patients' needs with the care team and coordinating plans of care.

The role requires a Master's degree in Social Work, reliable transportation, and computer proficiency; hospice/healthcare experience is preferred. This is a PRN opportunity based in Fort Worth, TX.

Qualifications

• Reliable transportation and a valid driver's license.
• Hospice or healthcare experience preferred.
• Proficient computer skills required.

Responsibilities

• Assess the psychosocial needs of the patient and family members.
• Provide therapeutic counseling and emotional support to patients and family members, and education on end-of-life issues.
• Contribute to and execute the Plan of Care with the family/patient.
• Record daily documentation of visits and contact in accordance with Policy.
• Provides referrals to community resources.
• Performs other duties as assigned.

POSITION SUMMARY

To provide therapeutic/emotional support, education, and resource information to patients and their families; Advocate for patients’ needs/wishes with team members, family members, caregivers, and other providers.
