Job summary

Community Healthcare of Texas is seeking a Social Worker in Dallas to provide therapeutic support, education, and resource information to patients and families at end-of-life. You will advocate for patient needs, document visits, and collaborate with the care team to execute the Plan of Care.

The role requires a Master's in Social Work, driving eligibility, and healthcare experience is preferred. This is a full-time, on-site position in Dallas, TX.

• Proficient skills using a computer are required.

Responsibilities

• Assess the psychosocial needs of the patient and family members.
• Provide therapeutic counseling and emotional support to patients and family members, and education on end-of-life issues.
• Contribute to and execute the Plan of Care with the family/patient.
• Record daily documentation of visits and contact in accordance with Policy.
• Provides referrals to community resources.
• Performs other duties as assigned.

Community Healthcare of Texas is currently recruiting for a Social Worker.

POSITION SUMMARY

To provide therapeutic/emotional support, education, and resource information to patients and their families; Advocate for patients’ needs/wishes with team members, family members, caregivers, and other providers.

ESSENTIAL FUNCTIONS

• Assess the psychosocial needs of the patient and family members
• Provide therapeutic counseling and emotional support to patients and family members, and education on end-of-life issues
