Job summary

Pathways Hospice & Transition Home Healthcare is seeking a Social Worker (Hospice) for a PRN shift to support patients and families in the North/Northwest service areas. You will work with nursing to ensure Do Not Resuscitate orders are accurate and assist with advanced directives.

Must hold a Bachelors or Masters in Social Work and be licensed in Texas, with at least two years of supervisory social work experience in hospice or related medical home health settings.

Qualifications

• Bachelor's or Master's degree in Social Work required.
• Must be licensed social worker in Texas.
• Minimum two years of social work supervisory experience in hospice or related medical home health field.

Responsibilities

• Plans, develops, organizes, implements, evaluates, and directs the social services program of the hospice patient.
• Interviews resident and family during admission to complete social history and assess patient needs.
• Explains and obtains Advanced Directives with residents and families.

Social Worker (Hospice)

Service Area for Coverage: North,Northwest

The Job:

Support the successful execution of our mission and Core Values by improving or maintaining each patients’ ability to control everyday physical, mental, emotional, spiritual and psycho-social needs.

Responsibilities:

• Plans, develops, organizes implements, evaluates, and direct the social services program of the hospice patient
• Interviews resident and family during admission process to complete a social history and assessment of the patient to identify problem areas, and to develop rapport
• Responsible for explaining and obtaining Advanced Directives (Directive to Physician, Medical Power of Attorney) with residents and families.
• Works with Nursing department to ensure accurate Do Not Resuscitate orders are obtained for patients
• Develops and implements policies and procedures for identifying the medically-related social and emotional needs of the patient.
• With qualified supervision or with the appropriate license, applies Social Work Theory, knowledge, methods, ethics and the professional use of self to restore or enhance social, psychosocial, or biopsychosocial functioning of patients.

Must Haves:

• Must possess a Bachelors or Masters degree in Social Work, Must be a licensed social worker in the State of Texas.
• Minimum of two (2) years of social work experience in a supervisory capacity in a hospice or other related medical home health field
• Must have outstanding organizational skills andpossess the ability to deal tactfully with associates, patients, family members, visitors, government agencies/personnel and the general public.
• Proficient in the use of computer systems (Word, Excel, Electronic Health Record system, etc.)
• Must be in good standing with the Federal and State Government
