Job summary

Community Healthcare of Texas in Fort Worth is seeking a Social Worker to provide therapeutic and emotional support to patients and families, educate on end-of-life issues, and connect them with useful community resources.

You will advocate for patients' needs with the care team, contribute to the Plan of Care with the patient and family, and ensure daily documentation is completed in accordance with policy.

Qualifications

• Master's degree in Social Work required.
• Hospice or healthcare experience preferred.
• Proficient computer skills required.

Responsibilities

Community Healthcare of Texas is currently recruiting for a Social Worker.

POSITION SUMMARY

To provide therapeutic/emotional support, education, and resource information to patients and their families; Advocate for patients’ needs/wishes with team members, family members, caregivers, and other providers.

ESSENTIAL FUNCTIONS

• Assess the psychosocial needs of the patient and family members
• Provide therapeutic counseling and emotional support to patients and family members, and education on end-of-life issues
• Contribute to and execute the Plan of Care with the family/patient
• Record daily documentation of visits and contact in accordance with Policy
• Provides referrals to community resources
• Performs other duties as assigned

POSITION QUALIFICATIONS

• Master's degree in Social Work required
• Reliable transportation and a valid driver's license
• Hospice or healthcare experience preferred
• Proficient skills using a computer are required
