Job summary

Heart to Heart Hospice in Indiana is seeking a Social Worker to assess psychosocial, grief, financial and community needs of hospice patients and families, and to document care plans.

The ideal candidate holds a Bachelors or Master’s degree in social work, current state licensure, a valid driver’s license, and at least one year of clinical social work experience; hospice experience is preferred. We offer competitive pay, benefits, and mileage reimbursement.

Qualifications

• Bachelor's or Master's degree in social work from a State accredited program.
• Current State license required in accordance with State regulations.
• One year of experience as a Social Worker in a clinical care setting required; hospice experience preferred.

Responsibilities

• Assess hospice patient to identify psychosocial, grief, financial, environmental and community needs as evidenced by POC documentation, clinical records, team meeting minutes and community resource referrals.
• Provide individual patient/family counseling, crisis intervention, information and assistance with preparation of advance directives, funeral planning issues and transfer of responsibilities relative to legal and fiscal issues.
• Facilitate community resources based on patient/family needs.
• Accept clinical assignments that are consistent with education and competence to meet the needs of the patients.
• Perform on-call responsibilities and provide on-call service to patients/families as assigned.
• Timely submission of all documentation.
• Performance of other duties as required.
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures.
