Job summary

Heart to Heart Hospice in Houston, TX, is seeking a compassionate Social Worker to assess hospice patients and identify psychosocial, financial, environmental needs. You will provide individual and family counseling, assist with advance directives and funeral planning, coordinate community resources, and ensure timely documentation while maintaining compliance with laws and policies.

The role requires a degree in social work, state licensure, a driver's license, and at least one year of clinical

Qualifications

• Bachelor's or Master’s degree in social work from a State accredited program.
• Current State license required in accordance with State regulations.
• Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance.
• One year of experience as a Social Worker in a clinical care setting required, hospice experience preferred.

Responsibilities

• Assess hospice patient to identify psychosocial, grief, financial, environmental and community needs as evidenced by POC documentation, clinical records, team meeting minutes and community resource referrals.
• Provide individual patient/family counseling, crisis intervention, information and assistance with preparation of advance directives, funeral planning issues and transfer of responsibilities relative to legal and fiscal issues.
• Facilitate community resources based on patient/family needs.
• Accept clinical assignments that are consistent with education and competence to meet the needs of the patients
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
