Job summary

Heart to Heart Hospice is seeking a Social Worker to assess psychosocial, grief, financial, environmental and community needs of patients, documenting in POC and collaborating with the care team.

You will provide individual counseling, crisis intervention, assist with advance directives and funeral planning, and coordinate community resources to support patients and families. The role requires a MSW or BSW and a valid state license, with experience in clinical care and hospice preferred.

Qualifications

• Bachelors or Master’s degree in social work from a State accredited program.
• Current State license required in accordance with State regulations.
• Current Driver's License and reliable transportation, with insurance.
• One year of experience as a Social Worker in a clinical setting; hospice experience preferred.

Responsibilities

• Assess hospice patient to identify psychosocial, grief, financial, environmental and community needs as evidenced by POC documentation and records.
• Provide individual patient/family counseling, crisis intervention, and guidance on advance directives and funeral planning.
• Facilitate access to community resources based on patient/family needs.
• Accept clinical assignments within education and competence to meet patient needs.
• Perform on-call responsibilities and provide on-call service as assigned.
• Timely submission of all documentation.
• Ensure compliance with laws, Medicare regulations and policies.

What You Will Do:

• Assess hospice patient to identify psychosocial, grief, financial, environmental and community needs as evidenced by POC documentation, clinical records, team meeting minutes and community resource referrals.


• Provide individual patient/family counseling, crisis intervention, information and assistance with preparation of advance directives, funeral planning issues and transfer of responsibilities relative to legal and fiscal issues.


• Facilitate community resources based on patient/family needs.


• Accept clinical assignments that are consistent with education andcompetence to meet the needs of the patients


• Perform on-call responsibilities and provide on-call service to patients/families as assigned


• Timely submission of all documentation


• Performance of other duties as required


• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
