Job summary

Heart to Heart Hospice seeks a compassionate Social Worker to assess patient needs, provide counseling, and coordinate community resources for hospice patients and families.

You will handle advance directives, crisis intervention, and funeral planning issues while ensuring timely documentation and compliance with regulations. A valid state license and driver’s license are required, with hospice experience preferred.

Qualifications

• Bachelor's or Master’s degree in social work from a State accredited program.
• Current State license required in accordance with State regulations.
• Current Driver's License, reliable transportation, and provide ongoing valid auto liability insurance.
• One year of experience as a Social Worker in a clinical care setting required, hospice experience preferred.

Responsibilities

• Assess hospice patient to identify psychosocial, grief, financial, environmental and community needs as evidenced by POC documentation and records.
• Provide individual patient/family counseling, crisis intervention, and assist with advance directives and funeral planning issues.
• Facilitate community resources based on patient/family needs.
• Accept clinical assignments consistent with education and competence to meet patient needs.
• Perform on-call responsibilities and provide on-call service to patients/families as assigned.
• Timely submission of all documentation.
• Ensure compliance with local, state and federal regulations and agency policies.

What You Will Do:

• Provide individual patient/family counseling, crisis intervention, information and assistance with preparation of advance directives, funeral planning issues and transfer of responsibilities relative to legal and fiscal issues.
• Facilitate community resources based on patient/family needs.
• Accept clinical assignments that are consistent with education and competence to meet the needs of the patients
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
