Job summary

Heart to Heart Hospice is seeking a Social Worker to join our compassionate team in Houston, TX. This on-site role supports patients and families facing life-limiting illness with counseling, advance directive prep, and resource coordination.

Required: MSW or BSW, state license, valid driver’s license, and at least one year in clinical care; hospice experience preferred. We offer a sign-on bonus and comprehensive benefits including company car and mileage reimbursement.

Qualifications

• Must hold a current State license in accordance with State regulations
• One year of experience as a Social Worker in a clinical care setting required, hospice experience preferred

Responsibilities

• Assess hospice patient to identify psychosocial, grief, financial, environmental and community needs as evidenced by POC documentation, clinical records, team meeting minutes and community resource referrals.
• Provide individual patient/family counseling, crisis intervention, information and assistance with preparation of advance directives, funeral planning issues and transfer of responsibilities relative to legal and fiscal issues.
• Facilitate community resources based on patient/family needs.
• Accept clinical assignments that are consistent with education and competence to meet the needs of the patients
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
