Job summary

Heart to Heart Hospice is seeking a Social Worker to assess patients, provide counseling, and coordinate community resources. The role emphasizes compassionate care across families and patients dealing with life-limiting illnesses.

You will engage in on-call duties and ensure timely documentation within a compliance-focused environment. The position requires a social work degree, an active state license, a valid driver's license, and at least one year of clinical experience, with hospice

Qualifications

• Bachelor's or Master's in social work from an accredited program.
• Current State license as a social worker.
• One year of experience as a Social Worker in a clinical care setting.
• Valid driver's license and reliable transportation.

Responsibilities

• Assess hospice patients to identify psychosocial, grief, financial and community needs.
• Provide individual counseling to patients and families, crisis intervention and advance directive assistance.
• Facilitate access to community resources based on patient and family needs.
• Accept clinical assignments aligned with education and competency for patient needs.
• Perform on-call duties and provide on-call service to patients and families.
• Timely submission of all relevant documentation.
• Perform other duties as required and ensure compliance with laws and policies.

What You Must Have

• Current State license required in accordance with State regulations
• Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance
• One year of experience as a Social Worker in a clinical care setting required, hospice experience preferred

What You Will Do

• Assess hospice patient to identify psychosocial, grief, financial, environmental and community needs as evidenced by POC documentation, clinical records, team meeting minutes and community resource referrals.
• Provide individual patient/family counseling, crisis intervention, information and assistance with preparation of advance directives, funeral planning issues and transfer of responsibilities relative to legal and fiscal issues.
• Facilitate community resources based on patient/family needs.
• Accept clinical assignments that are consistent with education and competence to meet the needs of the patients
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
