A leading healthcare provider in New York City is seeking a Social Worker Lv II. In this role, you will perform psychosocial assessments, provide crisis interventions, and collaborate with a multidisciplinary team for effective patient discharge planning. Purpose:
The Social Worker should possess the qualities of independent functioning and self direction in the professional role and must have reached a level of development which requires limited use of supervision and / or consultation. Self-direction in addition to being reflected in individual case selection and treatment, should also include self-evaluation of one’s practice.
Specific Duties and Responsibilities:
The Social Worker Lv II is charged with and responsible for:
1. Interview patients and / or relatives in order to provide psychosocial assessment, to determine nature and source of problems and their inter-relationship with medical problems under study or treatment.
2. Provide casework services as well as supportive and crisis intervention counseling, to patients and families for discharge planning.
3. Confer with Medical, Nursing, and other professionals, in multidisciplinary rounds, to develop an assessment and care planning towards a safe and timely discharge for the assigned patients caseload.
4. Communicates with community agencies about provision of services. Initiates referrals on behalf of patients.
5. Keeps weekly statistics of case activities.
6. Recording of case material, and documentation in the Electronic Medical Record (EPIC), according to established guidelines and policies of CMS and the Department.
7. Access to EPIC, Allscripts, Outlook, ParachuteHealth.
8. Will be responsible for providing assessment and intervention on Rape, Domestic Violence, Elder Abuse, Child Abuse and Crisis Intervention.
9. Will have rotational Emergency Room/Maternity/Pediatric Units, Inpatient Discharges coverage to include Weekends, Evenings and Holidays.
10. Will participate in Social Workers weekend and holidays coverage for ED, Maternity and Pediatric on continues basis.
11. Will participate in Performance Improvement activities and monthly meetings and will be able to become a resource and role model for junior staff and caseworkers in practicing Social Work in Health Care Environment.
12. After cross training the Social Worker shall be able to cover all outpatient and inpatient units and Social Work programs within the hospital and its satellite clinics, to include Maternity/Pediatrics, Emergency Room, and training programs, etc.
For Assignment to Level II
1. Valid license as an LMSW issued by the NYSED; or
2. Valid license as an LCSW issued by the NYSED.
