9:00 A.M – 5:00 P.M
Purpose:
Under professional supervision and as a member of an interdisciplinary team, provides clinical social work services – individual, group and family – to adult and geriatric psychiatric patients registered in the Assertive Community Treatment Team
Specific Duties and Responsibilities:
1. Perform all duties in alignment with the mission, vision, values, and behavioral expectations of NYC Health + Hospitals
2. Conducts home visits, and/or telehealth as per protocols, participate in intake screenings to enroll new clients into program, participate in family sessions, visit client at the hospital, coordinate care with outside agencies, as needed; may co-lead groups, facilitate social activities for clients as needed.
3. Development of psychosocial assessments and social work goals through history obtained from patients, family members/significant others and other sources to whom patient is known
4. Provision of supportive counseling and psycho-education for patients, family members/significant others.
5. Advocates for patients, family members, significant others with community resources, i.e. benefit and entitlement programs.
6. Member of interdisciplinary team(s) responsible for the formulation of treatment plans during the course of treatment and discharge plans for returning/maintaining patients to community living.
7. Leader/Co-leader of patient group therapy meetings and/or community meetings.
8. Provision of appropriate care services for patients, i.e. housing, residential/outpatient treatment services, special needs, i.e. homecare services, etc.
9. Coordinates activities performed by the Caseworker who assists the Social Worker with provision of services to patients, family members/significant others, if applicable.
10. Provide most of the treatment in the community, or where the client may prefer.
11. Conducts AOT reporting weekly and service verification each month and as needed for AOT cases assigned.
12. Completes initial and update assessments in CAIRS reporting when due for assigned clients.
13. Demonstrates core competencies in designed areas of practice, including the Assertive Community Treatment Core processes, integrated mental health and substance abuse treatment, supported employment, family psycho-education and wellness self-management.
14. Other responsibilities pertinent to treatment and team functionality, including QI Projects to be developed and completed, as clinically indicated and assigned by the Team Leader/Supervisor.
15. Telework may be available upon completion of departmental probation. Telework/telecommuting may involve home visits but no return to the hospital/office.
Administrative:
1. Compilation and submission of monthly statistics and requested reports.
2. Completion of Quality Assurance monitoring forms monthly, if needed.
3. Attendance at monthly Departmental and unit meetings, if needed.
4. Attendance at conferences, seminars, workshops, training programs that will enhance knowledge and skills being utilized for patient care.
5. Department of Social Work Services Queens Hospital Center participant/ representative on intramural/extramural committees.
6. Complete treatment plans, psychosocial assessments, substance abuse assessments, progress notes, CAIRS, and all other paperwork that is required to successfully and competently treat the clients.
7. Develops and completes QI projects
For Assignment to Level II
1. Valid license as an LMSW issued by the NYSED; or
2. Valid license as an LCSW issued by the NYSED.
