Qualifications

Job Description

Purpose of Position:
This class of positions encompasses professional, supervisory or administrative work of varying degrees of
difficulty and responsibility in the operation of a social work department providing psychosocial and other
social work services to different types of patients across various settings. Ensures patient-centered quality
care and services in support of better outcomes and improved patient experience, in accordance with all
applicable local, state, and federal rules and regulations, and social work standards of care. There are five
(5) Assignment Levels within this class of positions; all personnel perform related work.

Responsibilities:

  • Completes Social Work Comprehensive Assessment for all patients asking for assistance, referred by others or in need of social work services. This is completed in accordance with departmental high/moderate risk criteria policy.
  • Conducts interviews with patients and/or family members(s)/significant others(s) to determine current concerns and continuing care needs.
  • Completes all appropriate social work documentation within designated time frames.
  • Provides a hand-off of psychosocial information to the next worker for patients transferring to other units to ensure continuity of care.
  • Provides supportive counseling, crisis intervention, and grief counseling to patients and their families/significant others as appropriate.
  • Assists patients with adjustment to treatment, including problems of depression, anxiety, denial and/or anger; noncompliance with medical regimen.
  • Devises/implements a social work treatment/continuing care plan (discharge planning) involving the patient, family/significant other and the interdisciplinary team. Documents this planning in the patient’s medical record in accordance to the departmental policy.
  • Provides information, referral and advocacy to community agencies.
  • Advocates and serves as a liaison for patients with community agencies.
  • Coordinates individual interdisciplinary case conferences as needed.
  • Provides task supervision to caseworkers when designed.
  • Provides direct/task supervision to social work students when designated.
  • Leads education and support groups for patients and/or families/significant others as appropriate.
  • Identifies, evaluates and designs proper safety plans for victims of crimes/abuse, i.e: child abuse, elder abuse, domestic violence, rape and sexual assault.
  • Maintains and completes statistics of case activities as outlined in the departmental policy.
  • Monitors his/her performance by reviewing medical records for appropriate documentation.
  • Attends individual, unit, departmental and intra/inter hospital meetings as mandated by the supervisor and/or department director.
  • Assists/participates in presentations, training and in-services as required.
  • Participates in the department’s cross-training program.
  • Coordinates and executes discharge plans for hospital patients.
  • Performs other duties assigned by supervisor as needed.
Minimum Qualifications

For Assignment to Level II
1. Valid license as an LMSW issued by the NYSED; or
2. Valid license as an LCSW issued by the NYSED.
