Lincoln Medical and Mental Health Center seeks a Social Worker II to provide comprehensive social work services, including discharge planning and individual counseling. The ideal candidate will hold a valid LMSW or LCSW license and possess a Master's Degree in Social Work. This role involves working closely with patients and families to ensure continuity of care and effective treatment plans.

PURPOSE OF POSITION:
Under general supervision, with moderate latitude for the exercise of independent judgement, in addition to performing the typical tasks of a Social Worker II. Provides social work services, discharge planning services and coordination of continuing care services by providing a variety of services such as individual counseling, family interventions, and community networking to patients serviced at this facility.
DUTIES & RESPONSIBILITIES:
• Coordinates and conducts quality activities within the department regarding social work practice on complex issues
• Acts as project leader for special studies and research projects. Collects and evaluates data related to vulnerable client populations. Prepares and presents reports to hospital or social work staff related to these service populations
• Trains and instructs social work staff in clinical social work techniques. Serves as a resource and role model for beginning social work staff in various aspects of advanced social work practice
• Formally orients and educates members of other professional disciplines by developing training curricula and presenting lectures covering workshops, seminars, etc.; on social work concepts and functions
• Conducts interviews with patients and/or family members(s)/significant others(s) to determine continuing care needs.
• Completes Social Work Comprehensive Assessment for all patients referred or in need of social services in accordance with departmental high/moderate risk criteria policy.
• Complies with social work documentation and it requirements.
• Provides psychosocial information for patients transferring to other units to ensure continuity of care.
• Provides supportive counseling, crisis intervention, and grief counseling to patients and their families/significant others as appropriate. • Assists patients with adjustment to treatment, including problems of depression, anxiety, denial and/or anger; noncompliance with medical regimen.
• Coordinates psychiatric admissions with other acute psychiatric care institutions.
• Devises/implements a social work treatment/continuing care plan involving the patient, family/significant other and the interdisciplinary team and documents on the patient’s medical record in accordance to the departmental policy.
• Provides information, referral and advocacy to community agencies.
• Advocates and serves as a liaison for patients with community agencies.
• Coordinates individual interdisciplinary case conference as needed.
• Provides task supervision to caseworkers when designed.
• Provides direct/task supervision to social work students when designated.
For Assignment to Level II
1. Valid license as an LMSW issued by the NYSED; or
2. Valid license as an LCSW issued by the NYSED.
Departmental Preferred:
• Master’s Degree from an accredited Graduate School of Social Work.
• Current LMSW or LCSW issued by the New York Department of Education.
• 0-2 years of experience in the social work field

• Performs other duties assigned by supervisor as needed.
KNOWLEDGE&SKILLS:
• Able to engage patient/family members/significant others to obtain pertinent psychosocial information and develop an appropriate social work/discharge plan.
• Able to utilize clinical skills to assess patient’s ability to function in the community.
• Able to communicate proficiently (verbally and in writing) in the English language
