PURPOSE OF POSITION
To provide social work services to patients in the CD/MSOP program

SUMMARY OF DUTIES AND RESPONSIBILITIES
• Provide direct clinical services to patients, including assessment, treatment planning, counseling, and case management, within the Outpatient Addiction Clinic.
• Collaborate with the interdisciplinary treatment team to develop, implement, and monitor individualized treatment and discharge plans.
• Address the needs of special populations (e.g., women, pregnant individuals, HIV-positive clients, individuals with co-occurring mental health and substance use disorders, and those involved in the criminal justice system) through appropriate, evidence-based interventions.
• Provide psychoeducation and counseling to patients, including harm reduction strategies, opioid overdose prevention, and the distribution of naloxone kits and fentanyl test strips.
• Assist patients in accessing community resources, including healthcare, housing, and financial assistance, to support their treatment and recovery.
• Support patients in navigating the process of third-party payor documentation, ensuring that financial and insurance-related information is accurately captured and communicated.
• Utilize evidence-based counseling techniques such as motivational interviewing and behavioral change models to enhance patient engagement and progress.
• Document all patient interactions, including assessments, treatment plans, progress notes, and discharge summaries, in compliance with hospital policies and legal requirements.
• Participate in interdisciplinary team meetings and case conferences to discuss patient progress and coordinate care.
• Collaborate with community-based organizations and external agencies to ensure comprehensive care and continuity of services for patients.
• Attend and participate in in-services, staff meetings, case conferences, huddles and other required training sessions to stay current with best practices and program updates.
• Maintain compliance with all relevant clinical standards, social work practices, and governmental regulations, including the reporting of suspected child abuse or neglect to the New York Central Registry when applicable.
• Maintain professional boundaries and confidentiality in all patient interactions and documentation
• Coordinate and conduct quality activities within the department regarding Social Work practice, Addiction Services and/or any other identified training needs/complex issues/cases.
• Adhere to infection control policies, emergency preparedness plans, and environmental safety procedures
• Contribute to the completion of monthly statistical reports for the Outpatient Treatment Program and NYC Department of Mental Health.
• Participate in Performance Improvement (PI) and Quality Improvement (QI) initiatives related to social work practice and patient care.
• Maintain professional boundaries and confidentiality in all patient interactions and documentation
• Participate in interdisciplinary performance improvement projects and complete monthly productivity and other statistics as requested. Ensure the timely completion of required reporting and logs.
• Escort patients for discharge when appropriate and/or indicated by a supervisor.
• Perform other duties as assigned by supervisor.
For Assignment to Level II
1. Valid license as an LMSW issued by the NYSED; or
2. Valid license as an LCSW issued by the NYSED.
