Qualifications

• Master's degree in social work or related discipline.
• Unrestricted Minnesota social work license (LGSW/LISW/LICSW).
• Experience with electronic health records or similar software.
• Ability to work with interdisciplinary teams and diverse patient populations.

Responsibilities

• Identify patients needing social work assessment and intervention.
• Interview clients and perform bio-psychosocial assessments.
• Complete documentation for accurate billing of clinical services.
• Design and follow through on care plans with interdisciplinary teams and community resources.
• Provide supportive counseling and crisis intervention under LICSW supervision.
• Coordinate discharge/transition planning and care management.
• Refer clients to community services and advocate for access to resources.
• Document psychosocial assessments, care plans and progress in the EMR.
