Job summary

The University of Iowa Health Care, Care Coordination Division is seeking a Social Worker II/III to provide direct social work services including assessment, treatment, interventions, and recommendations to facilitate movement through the continuum of care to patients and their families. This role collaborates with an interdisciplinary team and may include group therapy and program development.

Shifts are part-time with on-campus work.

Qualifications

• Bachelor's degree with a major concentration in Social Work or related Human Services field.
• Master’s degree with a major concentration in social work (for SW III).
• Excellent written and verbal communication skills.

Responsibilities

• Develops and implements plan of care responsive to patient/family concerns and clinical impressions.
• Performs social work assessments with patient and/or family involvement.
• Provides information, consultation and professional support to interdisciplinary team members.
• Advocates for the social work role with health care team members.
• Develops and facilitates groups for patients and/or significant others as appropriate.
• Provides therapy, educational, and/or support group services reflecting diagnosis and age-related issues.
