Qualifications

• Master’s degree in social work or related field.
• Current LICSW licensure by Minnesota Board of Social Work.
• Board-approved supervision experience.

Responsibilities

• Identify patients needing social work assessment through high-risk screening and referrals.
• Interview clients, conduct bio-psychosocial assessments, and document for billing.
• Design and monitor long/short-term care plans with the team and resources.
• Provide supportive counseling and crisis intervention as needed.
• Deliver care coordination, advocacy, and connection to community services.
• Possibly supervise LSWs/LGSWs and supervise interns for licensure.
• Contribute to discharge/transition planning and family advocacy.
• Explain policies and programs to clients; maintain EMR documentation.
• May conduct field visits to clients’ homes when required.
• Run psychoeducational/support groups and participate in multidisciplinary initiatives.
• Support housing, employment, financial assistance, and behavioral health needs.
