Job summary

WSFSSH is seeking a Social Worker I to work autonomously while developing 1:1 relationships with residents to provide a comprehensive range of social work services that promote safe and independent living in the community.

Qualifications

• MSW or MA in human services with a minimum of two years social work experience.
• Fluency in written and oral English required; bilingual a plus.
• Experience with older adults and people with mental illness and/or substance use preferred.

Responsibilities

• Assist residents with obtaining and maintaining appropriate medical and mental health services, entitlements and home care.
• Establishing and/or maintaining family and social relationships; budgeting and income maintenance to ensure on-time rent payment.
• Provide crisis intervention services and client appointment escorts; conduct individual counseling.
• Assist residents in participating in socialization activities through in-house and community referrals.
• Follow up with residents and other service providers during hospitalizations or short-term rehabilitation stays.
• Oversee discharge planning and complete necessary forms and applications to other facilities.
• Provide group work services for recreational, educational and clinical needs.
• Participate in intake of prospective residents and share progress information with staff per confidentiality policies.
• Maintain thorough case records within required time frames.
• Attend supervision, team and staff meetings; perform other duties as assigned.
